An Organic and Healthy Approach to Parenting
Savvy Wellness Prenatal & Child Care Coordination is an innovative and organic program designed to help expectant moms and their babies stay healthy during and after pregnancy. We provide education, intervention and guidance, along with great incentives. This program is encouraged but voluntary for Medicaid members.
Prenatal care coordination is a Wisconsin Medicaid and BadgerCare Plus benefit that helps pregnant people get the supports and services they need during and right after pregnancy. The goal of prenatal care coordination is to connect pregnant people to medical, social, and educational services to improve birth outcomes.
Care coordination is a process that involves linking children (to age seven) and their families with health care, community supports and services in an effort to achieve good health.
Savvy Wellness Prenatal & Child Care Coordination believes that mothers' physical and emotional wellbeing is of utmost importance for not only a successful transition into motherhood, but for both your and your baby's long-term health. We provide mom/baby resources that are safe, eco-friendly alternatives to traditional baby products - which are often riddled with petrochemicals and synthetic fragrances. Savvy Wellness promotes nutritional options of organic and healthy food options along with safe, chemical free, non toxic mom and baby goods.
Medicaid
This program is intended for Medicaid members. A Forward Health Card is required for enrollment. We will assist applicants obtain their insurance identification cards if needed.
Are you currently pregnant?
Clients who enroll in Prenatal Care Coordination (PNCC) Services must currently be pregnant at the time of enrollment. Please plan to have proof of pregnancy available for enrollment. Other acceptable proof of pregnancy items are a doctor provided ultrasound, or pregnancy statement from an OBGYN and health clinic.
Do you have a newborn?
We welcome mom's and babies up to two months old. Babies over two months are eligible for Child Care Coordination (CCC) Services.
Are you currently working with another agency?
Child Care Coordination is an option for families enrolled in PNCC until their children turn seven years old.
If you would like to transfer from another agency we will provide assistance with obtaining your previous file and information.
